Abstract
Numerus reports and apperered in the literature citing the danger of pregnancy at 40 years and over. 89 cases of pregnancy at 40 years and over at the department of Obstetrics and Gynecology, Presbyterian Medical Center, Chun Ju, from Aug. 1,
1987
to
Jul, 31, 1992, were compared with randomly chosen 166 cases of pergnancy between 20 years and 30year during the same period.
@ES The result were as follows.
@EN 1) There were 89 cases of pregnancy at 40 years and over among total 19588 deliveries and the incidence was 0.45%.
2) There was not signficant difference in annual incidence.
3) Maternal age was distributed from 40 to 46 years. The majority(64.0%) was in the age group of 40 and 41 years.
4) There were significant increases in paragravida and weight of pregnant woman, but significant decreases in gestational age and length of labor.
5) Among antepartal complications there were significant increases in preterm labor, preeclampsia, placental abruption, intrauterine fetal death, cord prolapse and missed abortion in older age group.
6) Concerning the mode of deliveries, abdominal delivery was 41.6% in older age group and significantly increased when comparing with young pregnant women.
7) Among the indications of cesarean section, the highest incidence was previous cesarean section state and the next was elderly primiparagravida.
8) After vaginal delivery, laceration of uterine cervix was significant increased in older age group.
9) The were no difference in birth weight and macrosomia but significant increases in prematurity and low birth weight.
10) There were no difference in neonatal sex and congenital anomaly.
11) There were no difference in 1 minute and 5 minute Apgar score, but still-birth was significant increased in older age group.
12) There was significant increase in perinatal mortality in older age group.
